Is there such a plugin? I've seen this website with the post title as the watermark. All featured image of his posts has a watermark of the title post.
It would be pretty easy to build a plugin that does that I think, as its just a CSS color bar with text on it, and yes, could be in real time. You could even just use a code snippets plugin, and put the js/css in yourself

code snippets, i'm not sure its live at the moment on WP as it showed a false positive flag, and got taken down but has been patched and is available if you search for it on github
