In the example you give, it’s either a very easy keyword, or the page has backlinks that are hidden by the major SEO tools. This will almost certainly be PBNs.
Another situation could be that the page has lots of internal links from powerful pages on the domain. But as the DR is only 4 this seems unlikely.

Hidden PBNs for sure.One of my competitor rank on #1 spot for so many keywords with 0 backlinks and 0 referring domains.
