I've been messing around with my Kamikaze twitter accounts (accounts created knowing they will soon die) recently and have had some success with this - thought I would share. Click through rates are high, and my earnings per account created have gone up by about 60%. I do this in an automated way across several thousand new accounts per day, so I'm not sure what someone doing this manually could pull off, but the idea is very twistable.
It turns out that even though it takes days for new accounts to appear in search results, even brand new accounts can send @ replies which will instantly appear to their intended recipients.
So, for example (this is not one that I've used - just an example), you create an account and instantly tell a bunch of people that their profile pic makes them look fat and that they need Acai Slim immediately - with your affiliate link

Obviously don't direct link this kind of thing - fake your referrers and make the link that appears on twitter go to a domain other than the one your affiliate network will see as your referrer (in case the network/advertiser googles your domain and sees a whole bunch of "Wow you're a fat b*tch" tweets linking to your domain
).
Whatever your message is, it needs to be something that would really register with someone even if told to them by a complete stranger. In this example, a fair percentage of people are, of course, actually fat. This statement will register with them, even if they are insulted.
Of course the account will soon die, but if you do this during peak hours you'll make some $$$.
