On Instagram, the first comment is the top comment, so I am wondering if anyone is able to create a program which will immediately post a comment the very second an instagram post is posted, so it will be the first comment.

I know a guy who can do that: Mark Zuckerberg.
Other than that, it cannot be done "the very second", because Instagram has no "watch and notify in real time" feature, but you can get close for accounts you follow.
