How it looks like when Marketing Agency run a lot of accounts? Does Instagram know and accept it or they also run it with proxies etc?

Very high chance they use a program like massplanner/followliker and proxies

Depends if those are bots or humans behind the accounts. It would have to go against their TOS

Agencies use Hootsuite, Sproutsocial, Percolate, BrightCove, Agora Pulse, and others. These are not cheap when you have more than 5 accounts and up to 100 users on each. But they are some of the best. Total control and reporting.
