Depends on how the owner is good to monetize it. He can sell pretty expensive shoutout (really expensive) but with care. He can't just "advertise" someone random, because he has to maintain the page credibility, so it's not always that easy. Anyway, even with just some shoutout, he can make nice money.
If he has a website where he sells digital/physical products, then it's more money. He can also create his own product/course, so he has more flexibility. 1M page can generate you decent money

Now i understand it why the famous pages are always promoting smaller ones. Because there is money in between ... Lol was in the air
Lots of factors go into this... Niche, engagement etc..
Shoutouts are likely your best earner with a 1m following.. but never DM directly from your main account, create a fanpage for your main account built it up to 5k-10k and use this to contact potential clients. Have an email in your bio also. At 1m tho chances are people are already going to be DMing you requesting shoutouts.
Affiliate marketing is also a viable option but will be very dependable on your niche and it's pivotal that your product is quality otherwise you will lose your creditability.

Instagram Page owner can earn from an advertisement of products, from an individual image post these are the source of income of a page owner. Income depends on post charges.

Yes it really depends on how you monitize it. if you're good at monitoring I think that will make a good earning in the future 
Why I shouldn't dm people directly from my main?
Might be against Instagram policy to sell shoutouts not sure, but definitely a way to keep safe from unwanted trouble.

You can start charging after 10k depending on your engagement but people are never going to stop asking for free shit!
