I face(d) this more than I could handle.
My way of dealing with it is the following :
Make an EXHAUSTIVE list of things you NEED, WANT, and COULD do.
Doesn't matter if it takes you days to complete the list. Just do it.
Then, organize the list into the things that you really want to do and are comfortable doing. You will know in the first second which these are. For example, I can't stand copywriting/newsletter creation/banner creation anymore. I'd rather picking my nose everyday than create ONE single banner in photoshop. Even if it's the easiest thing for me.
So you now have your list arranged into things you will do by yourself, and things that should be outsourced.
Out of all this mess, cos it's still a mess, you must find the group of things that will lead you to creating something for either the long term, or the most profitable way ( long-short tem it really doesn't matter here ).
Start with ONE project at a time and make it as perfect as you can. Outsource the work needed to be done by others then set up to THREE goals for that project. For example : ranking goals, income goals, further additions goals etc.
Then take a break and start over for your next project, while doing maintenance work on the first. And on and on.
This might take a year or even two if it's something important and not a B-list blog or something, so you have to convince yourself that you can do it and it's worth it otherwise you're wasting your own time.

I also found that hiding from some people from FB/real life intentionally made me work like a maniac. Had no distractions and could actually not speak to the phone or lurk on FB. Turn off all distractions and focus on your long term target doing short-term steps one at a time.
