I need very simple page ( button opens small window)

Hello guys,
I want to build a simple page , there will be a button on the page.
when visitor clicks on the button there is a pop up window that he needs to choose between 3 options ( radio buttons) and that's it. how can i do it ? i can install wordpress but what plugins need to be installed for this purpose? it's very simple

If you use wordpress, you have to know wordpress hooks and they're very broad topic, you can't grok on the first day. Just use plain HTML, and some PHP if you need save the data.
