I need to replicate this site

I need to replicate a site (well...I need to replicate it's function... I don't want to just rip it off totally!)

It needs to be pleasant on the eye and use similar graphics.

I don't know what type of skill is needed when I go post on somewhere like oDesk or eLance.

It would be hosted on our VPS on it's own domain.

I'd appreciate your thoughts on how to move forward.

If you are looking for help / information on how to solve it the you should go to another section like programming or web design

If you want to outsource this to oDesk or elance the skills you should look out for are:

HTML + CSS for the page design (Ask for a responsive design here, because it'll show up on mobile too)
JavaScript for the Browser, OS etc. details
PHP or Java or Ruby or Python for the form

Such a site is super cheap to make and should be done < 1 day. You'll find many freelancers that are able to do this because this is very basic stuff. I'd personally wouldn't go with oDesk or elance since this project is simply too small. Better chose sb. here on BHW or maybe even fiverr.
