I need a 3 page MS Word document converted into a gravity form.

I have a friends 3 page employment application written in Microsoft Word. They have gravity forms installed on their Wordpress website and want the application as a gravity form. I don't have time today to do it, so id like to hire someone to recreate the application in gravity forms and then export it. Then send me the exported file and I can install it on my friend's website. So you must have a WP installation and gravity forms already installed to complete this task. You will be given no login details. You must have your own installation of gravity forms already. Not sure what to offer because it isn't hard so offering $5 +/- obo via btc or pp. If its too low please let me know your offer via PM.

Must know how to use gravity forms and all its features. (Checkboxes, radio button, line text, etc)
