I used hreflang tags on my website, but when I search "openx ad server" the German page shows up in Google, does anyone know why this is happening or how to fix it, thanks!

Keep in mind hreflang tags are optional for search engines. They do not have to follow your instructions.
With that in mind, most of the time they do, because they want to provide results that their users are searching for in the language they are searching for.
So, It's best to go through all your pages that link to the page your having issues with.
If any page referring to the page your having issues with is not also of the same hreflang tag, you will experience this problem.
You need to check all the internal links to the page, go through them, inspect the tags, make sure link A that points to link B both use the same language reference tag. You may have a typo, you may have missed a tag, or something of that nature. Even then, this can still happen.
Also check and see what language you are targeting within search console.
 
what does hreflang tag do? they prevent Google and other search engines to translate website content?
 
Its a piece of code you add to a website to define a geography and language reference and it's used to describe the content of the site's language and geography.
Search engines read this code when they connect to the site and use it to display accurate search results to other people defined by your language and geography reference in the code snippet. Hopefully.
But it's optional, and the search engine has other ways of tracking it's users, and can still feed them a different spotlight of results based on other factors.
The search engine may know you speak multiple languages because of various other tracking methods, so it chooses to over-look the tag and show you results in multiple languages.
 
Have you tested your implementation? Some tools, plugins don't implement correctly. Try the site hreflang.ninja to test or cross reference with this tool aleydasolis.com/en/international-seo-tools/hreflang-tags-generator/

Check the following:
  1. Are you using Google.com or Google.de when seraching?
  2. Does your Search Console locally targeted for every asset you have?
  3. Are your content created originally in German language and you google-translated it to English later?
  4. In your hreflang tags, are you sure you using the right Country & Language codes?
I think you can manage your way in this answer and if not - read Google targeting guidelines and make sure to implement correctly.
