How to write a Health Disclaimer?

OK, I'm about to finish my website for my health e-book.

Unfortunately, I do not know how to write the "Terms of Use" and "Privacy Policy"... anyone can help me here?

I've manage to did the Disclaimer part.. I'm just left with these 2.

Can I just go to my competitor's website and copy-and-paste their Terms of use and Privacy policy to my website? Of course, changing the names and such to my website or my name.
