How to make a second Adsense account?

My adsense account attached to my youtube account recently got banned.

I am willing to start over on a new adsense/youtube account, but im interested to know the precautions to not get removed by the auto-detect system.

From what I gaver, you need to:
-Sign with new home address
-Different name
-Different Youtube
-Different IP when logging onto my acc

but do i need to use a different bank account to the one I previously attatched to my 1st Adsense acc? (It never got approved in the end, so i was wondering if i could use it a second time).

um yeah i just used my sisters info....

If you are able to get partnered through a network such as TGN they can 'un-ban' your adsense account.

mine got banned to, I never bothered making another.

No, they can't. What they do is link your account into their CMS which is tied to their AdSense. Which doesn't require your AdSense since they collect the money then redistribute it.
