How to Get Approved by Amazon Affiliate?

It is now my third attempts in getting approved with Amazon Affiliate.
The review site I was building is about cooking appliance. It took me more than 1, may be 2 months to write all 15 pages of content. Most of them are 1000 words+.
Turn out that the day before yesterday, I did get a sale from it. I thought they will approve my application this time.
But, sadly as always, when checking email this early morning, they just denied my application.

I can't figure out what I was doing wrong. I actually include Privacy Policy, Term of Service and Amazon Affiliate Disclosure page. Besides that, I also not include the price detail or copy the amazon original content.

However, I'm using EasyAzon v4.0 for my site. Some people told me that, Amazon doesn't allow me to download their image and upload to the site. You must use the original image link in the post.
Is that statement true? Does it affect the approval consideration?
Because I find out that the Product Image link, which imported by EasyAzon, follow this structure : "mysite.com/wp-content/uploads/2015/04/414QJQjyqL1.jpg"

Maybe you can try looking at your top competitors sites in terms of the website structure and stuff. See what your site is lacking and improve on it.
 
I won't guarantee this will work but it's how I got approved. I used webicator theme which I have shared in the download section http://www.blackhatworld.com/blackhat-seo/templates-themes/712570-get-webicator-wordpress-theme.html

All I did was install the theme and add the sample products by following the PDF instructions and I got approved. It's worth a shot if you can't find anything else

Again, no guarantees but it worked for me. It's also a pretty cool theme depending on your niche

Do you use the same images and products description from amazon?
