how to find and install a survey widget on a site?

Hi,

I read a great thread here on "Massive Traffic & List Building" by xoskillet.

"they are presented with a widget that requires them to fill out a survey"
Can someone tell me where i can find a survey widget?
Do i need to find a company that provides this to websites? How?
