How to create qize like this?

I was doing some research to build a quiz site.
Then I found this site creating super simple quizes with 1 question.

method they use..
Basically upload a picture with some options to choose like 1, 2, 3

Ad answers as 1, 2, 3
You can decide the number from the image and you have to choose the number from answers.

Each answer have information or answers

My problem is how can I creat quiz like this. I want to know the steps to add information for every answer.
I tried to do this with wp quiz pro, buzzfeed quiz, hd quiz, quiz cat and some other plugins. Nothing worked.
