Google document will get you something similar...

Or a web designer/dev could do it for not a lot of money, its a couple of hours work!

They are using the service called Wufoo. You have to pay them about $19.95 a month just for these types of forms.

wufoo forms will be the easiest way.

Wufoo is the best one imo and they have a free version you could use up to 3 transactions or something like that (so you can pay them only after you made a few sales, pretty convenient)

That form was created with Wufoo. You can also integrate a paypal payment form to the order form. Much easier to track orders with Wufoo.
