How i this happening?

Lately there are plenty of site here got hacked by a hacker.
Ok they dont touch any of the files....fine.

But they just change the index.html to their own trademark.
The latest website is http://zamzarina.com/
and you can see the image trademark is still there.



Question:
How does they do it?
Go into the cPanel and change the index.html?

How to prevent this?

Nope its not mine.
Mine was hacked last week by the nick 'Stupid Hackerz'.
Thank God they didn't touch any of my file that time.
For that i'm using WordPress, and some told me that its a plugin breach.

Anyway this latest one is belong to a TV host in our country.
I think in a month there are more than 20+ cases like this keep on popping.

How they do and how to prevent ya?
