How do web firms sell a website for $10k+

Along with other things already said in previous replies, it's also a factor on who those services are targeted to.

Fortune 500 companies, movie production studios, election campaigns, etc. aren't likely to use a design firm that provides 5 page websites for $100. The $100 website designer is targeting your little mom and pop stores in local areas, and similar type start up internet businesses.

It's also important for the higher end design companies to have a track record of building high end sites for high end customers. If your last customer paid you $100 for his website, don't expect to go straight to a $10,000 job for your next one.

A client asked me for a WordPress theme. I don't like doing WordPress, so I tripled my old rate and asked for $1400 to make him go away. To my surpise,he said "sounds good" and paid it. Now I charge a minimum of $1400 for WordPress sites to small businesses and they have no problem paying it. I still don't like WordPress, though.

All those $100 bucks website's are built with CMS or template. And those $10k/site companies provide Custom Coding+Unique Design+Best Graphics & client support. Their main buyers or clients are companies while $100's client is outsourcers.

Reputation ,Reputation and Reputation.

Take a look at Tommy his PVA's arent better than from other suppliers but his great support and reputation are making that people are willing even to pay more.
At webdesign its also very important how the design convert traffic into cash as each additional % could make a lot of diffrence.
