Some proxy providers operate or obtain their proxies from botnets, which is a network of compromised computers.
Private proxy providers generally get their IP addresses from datacenters.
They can get them from many sources,
- Datacenter, borrowed, leased.
- ARIN of course.
- Big client with a big allocation
- Generous carriers / upstreams
- Block used already for other things (eg: mailing)
In my case, i have a datacenter and ive got a couple of B classes that i could use for proxy, im trying to contact Harro so i post an offer shortly, i could do 1 dedicated server with 4 C blocks and a 100 Mbps port for a reasonable price.

buy VPS or DEDICATED, more likely dedicated, because for VPS ips issued might be limited
then they ask 100 ips from datacenter where they host they dedicated
then all these 100 ips are directed onto dedicated server
then you put SQUID, or any other NIX or WINDOWS proxy software
then you can access internet via 100 ips through one dedicated
