How do I add ************** & a download link to a button?

If possible I want to be able to add ************** with my download button when which completed the download will pop up which will allow the person to download the file..

So when they click the "Download Now" Button the ************** will pop up first where they have to fill it out if they dont the download wont show..

If I need to explain this more please say, I don't know a lot about html so any guide is welcome or just the code
