How do CPA networks catch fraud?

Just curious about this, I know there is a bunch of different software available, but will a certain conversion rate or EPC trigger fraud detection? I know they check flash cookies, user agent, OS, IP, but what else?

How do people always get caught filling out their own offers? And can they tell if your getting traffic from Craigslist, if you have the person respond to the ad via email?
