If you only want to spend up to $500 then your going to need to do it yourself. Companies charge way more then that. Find the official government sites of the place you want to register and they might have the forms and the price of registering. Not every place uses LLC,but alot of places have a similar type of company under a different name.

I agree with teengenie,unless your banking several million a year you dont need a offshore llc. There is enough tax write offs in the u.s. Before you would need to go overseas. Plus the IRS looks more into offshore llc's nowdsys and you actually have to have management offshore as well.
No state in the United States is a tax haven,since you still have to pay federal taxes. Real tax havens are Ireland,Switzerland,Bulgaria etc. But if your going to create a llc in the United States,do it in Neveda.
