There are 2 kinds of penalties - manual and algorithm. Algorithm penalty means your ranking drops (sometimes significantly), but your website is still in the index. Manual penalty can be partial (for specific pages only) or for the whole website - in this case your website is out of the index, cannot be find there at all.

Difficult, but de-indexed domains are often penalized. Look for external links on popular sites and you would get to see a few among thousands.
