I'm using makemytrip affiliate and so far it is going good

I used http://www.hoteltravel.com/ for a year as they pay @ $12 per month for the first year even if you don't make any sales. I didn't make any sales and I don't use them anymore. It's a good start though especially for new sites.

Yes, but if you start getting conversions of a certain amount they stop your monthly payment. The $12 is simply for putting there ads on your site. Their conversion rate is very good. The email address is [email protected] and here is their affiliate PDF.
Did you try Travelpayouts? They have White Label, API, widgets. They are meta-search so have a lot of OTAs (including Agoda, Booking)
