Home Depot affiliate program- the main rules?

Hi, so not too long ago I finally got accepted into Home Depot's affiliate program. It said in the welcome message that it now requires us to report impression data through Commission Junction. How do you do that? Also, do you need a disclaimer on your site somewhere that you're affiliated with Home Depot?
