Highest Paying PPV/CPM Affiliate Program?

AdSense/PPC have a habit of lowering payout rates after the first couple months. Does anyone know of any high paying CPM/PPV affiliate programs that aren't hard to get approved for (like CasaleMedia/DoubleClick)?
