Hide that survey is a CPA ad..

I'm trying to put a survey on the site, but make it look like its only for site owners knowledge, not a CPA. Is it possible to make a CPA survey look like an information gathering survey for the site owner. If so how??

Ya I have found a way to incorporate it but does't the end direct them somewhere else, or can I make it direct them to another page on my site?

I have worked on making a fake form with the final text box the email and the submit image, cakesliced.

Why not do that for a survey. Make a survey that matches the one you are doing, the survey is all fake, only real part is the last part where they enter their email and submit. Below is what I did. That is a screen capture and the white boxes are real text areas, they really enter their name, street and state, it is just not recorded. The images may load slow since it is on a free hosting site.
