Idea is to make 1 page website with free ebook download, but download is locked and visitors would need to fill in the survey to unlock it.
Are there any single sale page templates available online? Which CPA network to choose and how to lock download button?

AdworkMedia is good for a start and has awesome free content locker that you can use to lock your downloads and people will need to complete surveys to get to the actual download

There are lots of content locking tutorials around, so simply use the search function if you need additional info and ideas

If this is how you planning to start CPA without even knowing the basics of basics it will take some time...
I don't see you getting accepted into AWM anytime soon...
1st steps you wanna take is....register domain name...buy hosting...install wordpress...put some copy/paste content make it look good,apply for adworkmedia and wait.if you get accepted come back here and i will help you set up the contentlocker and all ...
