I would work on a patient simulator with something like intelli j free eidtion, and Java. But to be honest, you'll get the most bang for your buck to start well with what i mentioned and a membership site with a bunch of developed products. Once you got the store done, and it takes you 10-30 mintues per day to do your daily socials, and marketing, then i would start an SAAS app, as if you don't have programming experiance, you will struggle at first and beat your head, wheasras you'll see quicker results from sales and things on fourthwall WHILE you develop your app.
