Hello everyone.
I hve handled these account verifications multiple times. Please note that Google has started advertiser verification programs. And they want to know who is advertising what and if it being advertised on someone elses behalf.
Most impacted advertisers are affiliates, resellers and agent businesses. Google if not satisfied, will suspend your account and that account and domain becomes unusable from any new ads account as it will be violation of policy.
Take the forms as much seriously and as much forthcoming as possible. I can help you fill the form or let you know if you will get suspended. But i will need full info like what are you advertising. Is it your own service or are you adveritising for someone else and more......

It is completely random whether you need verification or not. Much easier to just ignore and create new account.

Facing similar issue with my few non threshold accs. I'm creating new accounts regularly but was just curious to know about the google update regarding verification..
