Apart from the obvious reasons to get an account ban (same IP, cookies, browser bla bla), bans are happen due to:
1. Using different IP's when creating the account and logging in - if u use a US IP/Proxy to create the account, make sure the IP does not change - even if it is still a US IP, it wont help. U need the same IP or from a near location. For ex, if u use a NewYork IP to create the account, you cannot use, like 10 minutes later, a Los Angeles IP to login. If so, this is the reason they will provide you when chatting / emailing them:

2. Applying a CC that its GEO location different than the account creation & logging in. For ex, logging in from US, but the CC location is Germany.From our review, we believe your account may have been accessed by a third party to create online advertising without your permission. It is likely the third party obtained your personal account information elsewhere, and used it to access your Bing Ads account.
