Have you tried Amazon? They do have some good ad options available as well.
Some rotating ads, or dynamic ads, they have all of those.
Approval is also easy - just make sure you send an email to their affiliate department or communicate with the affiliate manager, and that most often would get your site approved.

media.net and infolinks.com are the ones that I would recommend to you. However, based on my experience, I would go for media.net over infolinks.com every time.

One cool thing about them: their system will ‘’learn’’ from your click-results, this is how they will optimize your earnings over time. I’ve seen rates going from $0.5 CPM and up to $30 per CPM. Of course it depends on the country and type of traffic you’re receiving.
