It would be useful if you provide us more details about your site or at least the niche you are into. 
Anyway, you will need to re-analyze your website from your on page analysis to your keywords, and to your SEO/backlinking campaign.
After doing this you need to correct (if there are any) the errors found on your site (upon running an onsite analysis using software tools like Google Websmaster or Traffic Travis). Then you may also want to have a check list of the good on page SEO practices and add these to your site.
After that of course, check your off page SEO campaign and see the backlinks (especially the quality).
It will also help if you are able to check your competitor's sites to see how do they their on page and off page SEO.
