    Cannot advise how to fix this problem without knowing any details - What packages are you running? Wordpress, Joomla etc. Also is this your own server (VPS, dedi) or is it shared hosting?

    it seems that they're only deface your blog..
    if that so, simply upload your original index.php from your template..
    hope you still have a backup

    yes shred hosting. But I wanna know the holes... is it my mistakes? I mean the holes is in my scripts or the hosting???

    Undoing the damage is not fixing the problem, what's to stop this happening again.

    Your scripts, so it's custom - probably your own fault then, you using databases? We need more information to help you. What's your sites address, what does it do etc, could be your permissions, SQL injection..

    It seem all folders containing domain is hacked. I did fix, but it happen again and again.

    All of your domains have been hacked? Who is your host - It may well be their problem.

    If he is right, you are lucky!

    Is there still your .html , .php and other data at your webspace???

    If yes, than just search for your old index.html/index.php (should be renamed, search after date) and rename it to index.xxx (xxx =php/html )

    example:

    You are hosting wordpress, your index is = Index.php , than the hacker renamed it to "index2.php" or "indeXXX.php" (XXX is random stuff) or he renamed it completely diffrent , than your have to "sort" after date, your index should be the 2nd newst file!

    Sounds like MySQL injection. With WordPress you have to be very careful how you configure your "Discussion Settings" or anyone can get in and do what they did to you.

    completely remove the index.html and re upload the new one

    Its probably an injection (method of hacking)

    Change your ftp passwords, cpanel logins etc. Plus, scan your computer for spywares...
