Well whats your niche?

Your post is quite vague.

But most probably your ad copy has to be optimized.
Use SKAGs or tighter ad groups. Depends on what you're running tho

Conversion set as Primary?and check if the tracking is correct also the conversion tag firing correctly?if tracking is wrong it is impossible to optimize
