Given Up Wth Site - Help Needed

So I've finally given up with a site that seemed to get slapped over a year ago. It had a very similar link structure to all my sites - manually built, relevant links - nothing much in the way of spammy links and was riding high until it got hit by the last EMD/ Panda/ Penguin update.

Despite spending a lot of time looking into the cause of the drop (offsite, onsite, links, duplicate content etc) I have never really found the reason why this one site got hit so hard.

Anyhow I have a lot of good unique content on there and a new domain ready to go.

My question is what are the steps I need to take to effectively 'rebrand' and move this to a new domain.

Can I literally restore the database and theme (It's wordpress) on the new domain?
How do I ensure the old site gets deindexed?
I guess I shouldn't 301 but instead just start building it up again?
Is this going to work?
Should I change hosting provider?

Is this going to work?

Any help or tips from anyone who has successfully done this would be greatly appreciated. :-)

1. Register the .org or .net with the same provider*
2. Make a simple index.html with the meta-tags noindex nof0ll0w, link an index2.html with the same meta tags. The index.html contains basically "see where we are now" and in the index2.html you put something like "For the following reasons (provider, blabla) we have moved to (insert link to your new domain)".
3. redirect your new domain to your existing folder (same provider, no further work)
4. Build backlinks, audience, the whole stuff, bank and thank me ;)


*Check, if the IP is known for spam, maybe this is your problem. In this case, change the provider.
 
I should have added.. I want to change the domain on this. So I guess it's the same except the first step?
