Freelancer needed for a simple landing page.

Hi I am looking for a freelancer to do the following ....

1. Set up a landing page questionnaire , 6-7 questions plus a drop down to allow the applicant to send me a photo or document and then forward to my email.
2. Once the landing page is completed , instal it to my domain.

the landing page will consist of a header banner and logo plus the questions with space to leave comments plus the photo download option.
