formmail to email - 2 pages

can someone post the code for making your formmail application form become 2 or 3 pages so spam is "less".. I'd like it to remember the initial page items like name, address and post everything to the form.

right now, I have a 1 page php form for all of this with some spam stoppers but I believe multiple pages will stop it.

Multiple pages will not help you. Try to use something like recaptcha.
