Form Creator to Article Plugin [Wordpress]

Hi guys. I don't know if you understood the problem by the title. I'm in a high need of a plugin that can create an on-page/on-article form with a Question and a Textarea for the answer, so the users could answer the questions, and after that all the content and also an image uploaded by the user to be published on an article automatically, as a draft in a certain category.

Is something that can do that ? I found a lot of Form Makers plugins but no one let me to publish the content as an article. On the other hand i found a lot of Guest Posts plugins that let me publish the article, but they have only "Title" / "Content" / "Author" but i have like 3-4 question and i need 3-4 textareas.

I made 3 paint drawings to make you understand better what I need and what I'm looking for.

This is the backend in the Wordpress Menu (how i imagine):





After i will insert the shortcode on a page, let's say, named Guest Posts, i will have something like this:
Here the user could answer to my questions.



And this is the final article generated:


Also if there are no plugins that can do this, but you you can modify and actual plugin or code this, please hit me with a price please, I really need this.

I think this can be done

Found two plugins that let me do that, but both of them are paid. Does anyone of you guys have "Formidable Pro" or "WP User FrontEnd Pro" ? Both of them let me to create custom fields and also post the article as Draft on a selected category.
