Fill this CPA offer and you could win... type of script

I doubt this title is clear enough so I'll give some details.

What I'd like to do is a simple website where ppl fill a CPA offer and then can leave their email and win something. I'd give a prize for each 100 CPA completed.

Do you think this would work? Also is there any script that would help me do this? And last question do you think there is a CPA company out there to support this?

I believe some of the cpa networks had something similar to this but in the form of raffle entries. Like they would give away an amazon gift card after x amount of entries.

well I dont want the CPA to give the prize.. I'd send the prize after a number of completed CPA.
