Well, many precautions can be taken, natural activity, proper cloaking(many people don't even use cloak) and websites that look like they were not built in 15 mins

Unfortunately it is not that simple anymore. Facebook is taking user feedback and reports seriously. The issue lies with human reporting as fake profiles/spam,etc. Account get block whenever such report exceed x threshold and when links get shared (get reported as spam).

All accounts running BH will Definitely get banned, its just a matter of when, there are stuff you can do to extend the lifespan.

True, but this problem is more often seen with group posting, with using paid advertising its more of the landing page quality, the ad copy, the cloak, and also if you don't hide those bad comments on your BH ads

I think for paid ads there is a report function as well. I know as I have reported an adult ad previously and the fanpage was taken down after my report within 15min. Facebook is quite efficient on this nowadays after the fake news incident during election.
