Had the same exact issue with a client based in the US two days ago. The client was white labeled to us. One of my staff (also based in US) entered the client's card into the ad acct. Instantly disabled. My employee filed the appeal. The end result was that the ad account has to pre-pay for ads for a period of time. The ad acct was activated by accepting the terms of pre-pay.
However, today the ad acct was disabled yet again. I'm certain it's the payment method, probably having been used in defunct ad accounts. Accept the pre-pay terms and you should get activated again right away. You may need to file an appeal to get the pre-pay terms to appear.
