exit survey scripts

I am looking for a method to collect some data from people leaving the site who haven't made a purchase. I want to achieve a few things.

1. Offer an incentive to complete a quick survey on exit
2. Pickup people who have added to cart but not made a purchase and then exited

I can create the survey in Survey Monkey but they don't include the exit script, does anyone know of a good solution.

If you know much Javascript it wouldn't be difficult to add the survey on exit - make a pop-up that shows the page on exit perhaps w/ a dialog asking them to fill it out

as for the shopping cart integration that would be a little trickier and depend upon what cart you are using for your site - might be a simple hook to add similar javascript to the top with a redirect to the checkout page or it might be a PITA...
