Ebay email addresses for surveys

I have an extensive list of past ebay customers. I was thinking of sending out an email list to all of them to rate their overall ebay/transaction experience. Anybody know of a good survey to send them or an alternative? I have looked through several aff programs but haven't really come up with anything that I don't think will just be discarded.
