Your full name or N/A, unless you have a company then of course you put the name of that.

I went to P33rFly last night after reading this thread. Applied about midnight, and when I got up this morning I was approved!

So I would say P33rFly, but it is the only one I have applied for.

CPA L34D is very easy, my application was approved immediately. I will try to get approved on the other networks here now and will post the results later.

In that box you should describ which kind of traffic you will have and how you get your traffic. Also they want to know how you want to make the user think he or she should complete the survey. You should describe what would be behind the gateway. Why wants the user to get access to your website.
