duplicated content on own url

hey is it duplicated content which is bad for seo if i move an article from /index.php to article.php?

or show the same form on every .php / .html file on my website?

1. If you are moving the article so it will no longer show on index.php, then there will be no duplicate content. You will likely lose any SEO value/traction that the article has gained as (I assume!) you're not redirecting index.php to article.php. But otherwise, no duplicate content because the article will only be showing in one place. Just use fetch as google on both pages in Search Console to make sure that Google knows the article has moved off index.php.

2. A simple form with some labels and inputs is unlikely to cause duplicate content because that would be stupid. If it a massive long form, with a ton of words introductory content, then it might be more of an issue.

If you have the same content on both Pages of your Website. Then you have to use rel="canonical".

If you are moving Page from one to another then in the no content Duplication Happened in Eye of Search Engine.

You should be fine with this. Think of it from a search engine point of view - it is part of your form and there is no reason why a form shouldn't be on all pages.
It doesn't detract from the user experience and tons of sites do this, it would be crazy to be penalised for it.

In case you want to use the same content on different pages, you've gotta make sure you add a canonical tag to all the pages (with duplicate content), pointing to the main one.

What's the main one?

The page which you want Google to see as the original/main page.
