It's a very difficult niche, it's a health niche about a disease. But I have this disease, and it's going to be a personal blog too, so hopefully it shouldn't be hard to rank.
At this point in time I haven't seen adding dashes into a domain name have any sort of negative effects on Google rank. In fact I've seen a website ranked incredibly well for a competitive phrase with two dashes in a row, (ie word1--word2.com). I did a bit of research on the site as well and it was not SEO'd all that great, it was mostly keeping it's top 3 spot with 3-4 years of domain age, a few back links, and the exact match domain.
I would definitely stick with .com/.net/.org other TLD's can get tricky really fast.
