Does twitter ever not unsuspend for aggressive following?

It's happened 3 or 4 times to me and I'm still fine.
I can't send links via DM anymore though but I think that because I DM'd about 100 people the same link to a youtube video in a day.
The account is my legit account though and totally whitehat apart from the fact I use bots for mass following/unfollowing.

I've been suspended quite a few times in the last 6 months. I find that if I follow say 500 throughout the day my account will get suspended, but if I follow 1,000 in one go every 24 hours, my accounts fine. Maybe as your account becomes more established they're less strict with it? But it seems to work for me.
I have heard of large accounts getting permanently suspended though, so it's not something I'd want to keep doing.

I've had accounts permanently banned on the first suspension.

Never for me lol i had an account banned 6 times in 1 week (experimenting) and all i had to do was just send them emails
