It doesn't matter but sometimes facebook verifies names so make sure you provide the name the account is under

Facebook do flag cards for payment previously but i am not sure for now. Did experience a period of time that when a ad gets banned, the same card can't be used for payment even if we change to a new fb account and page to start a totally new campaign

no problem as long as your card is legit and full payment to facebook

It doesn't matter who the card holder is but what matter is the legitimacy of the card and it must have balance to proceed the payment process.
